What is Unity PHM?

Unity PHM is a HIPAA-compliant, web-based population health platform that integrates with your EMR and PM systems. We offer customizable modules that you can implement independently to improve any specific area of your practice. Our clients use Unity to streamline everything from patient engagement and care management to clinical compliance, remote patient monitoring, and claims optimization.

What is Unity’s Vaccine Module?

Our vaccination module helps health centers manage paperless vaccine scheduling, administration, and documentation for large volumes of patients. It allows healthcare organizations to seamlessly track and manage patients through all phases of the vaccination process.

How long does it take to implement Unity Vaccine Module?

We can have you up and running on our Vaccine module in 1 to 2 weeks.

Does Unity Integrate with my EMR?

Unity is a certified integration in place with both Allscripts and eClinicalWorks; however, Unity can integrate with any EMR and PM system. Unity’s Vaccine module can also be used standalone without EMR integration.

How will users access Unity?

Your users can access Unity in any web browser; our Vaccine module also offers an iOS app that streamlines paperless vaccination documentation.

Can my organization use the vaccination platform in a drive-through setting?

Yes, the Unity Population Health iOS app enables care providers to manage both walk-in and drive-through vaccinations.

I am vaccinating thousands of patients a day, can the system handle the quantity?

Our system is designed and tested to handle massive patient volume—we are currently assisting our customers in a vaccination campaign across 12 counties in North Carolina.

Does the system automatically check whether a patient is new or existing?

Yes, the Unity platform ensures that there are no duplicate patients. If a patient is new, they will be updated directly to your EMR and PM.

Am I able to edit patient demographic information in Unity? Will the information automatically update in the EMR and PM once entered?

Yes, users have the ability to edit patient demographics in the platform. The information will automatically update in the EMR and PM.

How will patients change or cancel their existing appointments?

Patients receive a text containing a secure link with the next available appointment date, as well as the option to cancel.
